Rental dates, monthly rent, payment timeliness, property condition, noise complaints, lease violations, pets, whether the deposit was returned, reason for moving out, whether proper notice was given, and whether they would rent to the applicant again.
How long they have known the applicant and in what capacity, current employment, position and tenure, verification of the stated income range, and an assessment of character and reliability.
Whether the reference was verified, how cooperative they were, any red flags, free-text notes, an overall recommendation and who completed the form.
The reference relationship field covers a previous landlord, a current landlord, a property manager, an employer, a personal reference, or other.

Fixed options make two applicants comparable. Payment timeliness, property condition, deposit return, notice given and the final recommendation are all captured on a set scale rather than as prose.
Every fixed-option question includes a "Declined to answer" choice, so a non-answer is recorded as a non-answer rather than left blank.
